Referring to a dietitian
Although a referral is not necessary to see a dietitian, Medicare rebates are available to individuals on a Chronic Disease Management Plan organised through a GP. DVA Gold Card holders will be bulk billed for each consultation. Other funding and rebates may also be available to you. See below for more information.
Medicare rebates are available for patients referred to a dietitian as part of a Chronic Disease Management plan (formerly known as an Enhanced Primary Care plan).
Please make sure the patient has copies of these forms for the consultation:
- GP Management Plan (GPMP), Item 721 or 732 if a review
- Team Care Arrangement (TCAs), Item 732
- Follow up Allied Health Services for ATSI descent, Item 81320

Patients that hold a DVA Gold Card are able to access dietetic services free of charge.
Please make sure the patient has copy of this form for the consultation:

Some private health insurance will include rebates depending on your cover.
